Services

Live scan technology allows digitally scanned fingerprints to be submitted electronically to the Department of Justice (DOJ) within a matter of minutes and allows criminal background checks to be processed usually within 72 hours.

All results are sent directly, by DOJ, to the Requesting Agency within 2 weeks; only the Requesting Agency receives the results.

How The Process Works

  1. Start by booking an appointment online.
  2. Check-in at the UPD Front Counter, located on the first floor of the Student Business Services (SBS) Building, at your scheduled appointment time.
  3. After checking in for your appointment, the Live Scan technician will review your paperwork for the appropriate fees, and then you will be directed to pay at the Cashier's Office*.
  4. Go to the Cashier's Office at the Student & Business Services Building, Room 285, 2nd floor, to make the payment: (707) 826-6789.
  5. After paying the appropriate fees, return to the Live Scan office (with your receipt) to complete your fingerprinting.

*If your live scan is going to be paid by an on-campus department and you have a chartfield string, in most instances, you will not need to go to the Cashier's Office.

Costs, Fees & Payment

An application has two fees:

  1. A $15.00 rolling fee for Humboldt Students, Faculty, and Staff; or,
  2. A $30.00 rolling fee for Community Members; and,
  3. A pass-through fee(s).

The rolling fee is set by the Cal Poly Humboldt Police Department. The pass-through fee(s) are set by the U.S. Department of Justice (DOJ) and the Federal Bureau of Investigations (FBI).

Additionally, other various fees may apply if your application requires additional checks through other various State and Federal databases. Depending on your application, the fees can cost anywhere from $0.00 to as high as $84.00.

The cost of your application is unknown until the AORI code listed on your application is entered into the Live Scan device at the time of your appointment. Currently, the DOJ has set their pass-through fee at $32.00 and the Federal Bureau of Investigations has set their pass-through fee at $17.00. Please be advised that the DOJ and the FBI reserve their right to change their fees at any time without notice.

All fees will be calculated when the Live Scan Operator enters the AORI code. Once the amount is known, the Live Scan Operator will provide a payment form that can be taken up to the Cashier's Office for payment. The fees must be paid prior to submitting your fingerprint transmission. Please make sure to bring the appropriate payment method with you to your appointment to pay for your fees.

Location & Hours

Our live scan services are currently open by appointment only.

Live scan fingerprinting services are done through the Cal Poly Humboldt Police Department, which is located on the 1st floor of the Student & Business Services Building.

Live scan services are available by online appointment only and made in 20-minute blocks. Please schedule only ONE live scan transaction per appointment. If you require two or more separate live scans, please make an appointment for each one.

During the beginning of the semesters, we experience a high volume of applications for fingerprinting services. Late arrivals forfeit their appointment time. We kindly recommend that applicants arrive five minutes early if possible.

Method of Payment

Cash/Credit & Debit Card Payments

All applicants must check-in with the Live Scan technician BEFORE making any payments. The technician will provide the necessary payment form, along with the appropriate fees, once they have been matched up with the correct AORI code from the applicant's paperwork. All payments have to be made at the Cashier's Office at the Student & Business Services Building, Room 285, 2nd Floor.

On-Campus Billing

On-Campus Departments may request to have the rolling fee and the pass-through fee (i.e. DOJ/FBI fees) of prospective employees and/or volunteers, billed to their Department if they do not want their applicants to have to cover the fees associated with their application.

Policies

While we provide the live scan fingerprinting service, it is the responsibility of the applicant to obtain the appropriate form(s) from their outside Requesting Agency to be fingerprinted.

The Cal Poly Humboldt Police Department does not provide the DOJ Request for Fingerprinting Services Form.

Please obtain the form from the Requesting Agency and check the instructions from your outside Requesting Agency, or Campus Department, to make sure you have the proper information on the form.

Preparing For Your Appointment

Applicants must bring a valid form of government-issued photo identification to their appointment, and the identification presented must be current!

  • Unacceptable: Expired identification, DMV photo receipts, and Cal Poly Humboldt ID.
  • Acceptable forms of identification are:
    • Valid California drivers license or identification card,
    • Out-of-state drivers license,
    • Current passport,
    • Resident Alien registration card or immigration card (green card),
    • Military identification card, or
    • Mexican Consulate Identification Card (Matricula Consular de Alta Seguridad).

Applicants must also bring the completed Department of Justice (DOJ) BCII 8016 Request For Live Scan Service Form with the:

  • ORI Code,
  • Type of Application,
  • Job Title & etc.,
  • Agency Address
  • Contributing Agency and
  • Applicant's personal information

filled in prior to your appointment time. This form must be obtained from the person/agency requesting your fingerprint submission. We do not provide the DOJ form.

Live Scan Fingerprinting FAQs

  • How do I fill out the BCII 8016 Request For Live Scan Service Form?
     The information that you are responsible for on the BCII 8016 Request For Live Scan Service Form is the area field that starts with "Name of Applicant". For a detailed explanation of how to fill out your form, please download this BCII 8016 & 8016A Instruction Form in pdf format. Please read over this information carefully.
  • When will the results be available?
     The Department of Justice sends all fingerprinting results directly to the requesting agency only. Results are not provided to the University Police Department; nor, are they provided to the individual. Contact your requesting agency regarding questions pertaining to receiving the results.
  • My fingerprints were rejected, what do I do?
     If your fingerprints are rejected by the Department of Justice (DOJ), you will need to bring with you the rejection letter from the DOJ in order to have your fingerprints resubmitted. If your fingerprints were submitted at the Cal Poly Humboldt Police Department the rolling fee will be waived. If you were fingerprinted at another Live Scan agency, per Department of Justice Standards, you will be charged a rolling fee. All other fees will be waived by the DOJ.
  • My department/agency doesn't receive fingerprint responses from DOJ. How do we get access to the responses?
     Your department/agency needs to become an applicant agency with DOJ. If your agency would like to have access to criminal background information for the purposes of hiring, volunteer, or certification purposes, please go to the Department of Justice's Website to apply to become an Applicant Agency.
  • It's been two weeks since I had an applicant get fingerprinted, and I haven't received a response from the DOJ. What do I need to do?
     The following comes directly from the California Department of Justice website: “While most Applicant Agencies receive results of background checks electronically from the state Department of Justice, some must receive reports by U.S. mail in a process that can take a minimum of seven days. To check on the status of Live Scan fingerprint submissions, call the Department of Justice's 24-hour automated system at (916) 227-4557. There is no automated system for checking on paper fingerprint card submissions.
  • What info do I need to use the DOJ automated telephone system?
     To use the automated telephone system, you will need to have key information before making the call:
    1. the applicant's date of birth; and
    2. the 10-digit Automated Transaction Identifier (ATI) number that appears at the bottom of the Department of Justice form requesting Live Scan fingerprint background checks (the ATI number always appears in the following sequence: 1 LETTER; 3 NUMBERS; 3 LETTERS and 3 NUMBERS).

Because of the volume of calls received and time needed to research each request, there may be delays in responding to your inquiry.
